Overview

Mitchell Levy, MD, MCCM, FCCP,is Chief, Division of Critical Care, Pulmonary, and Sleep Medicine, Department of Medicine, The Warren Alpert Medical School of Brown University, where he is Professor of Medicine. He is also Medical Director of the Medical Intensive Care Unit at Rhode Island Hospital, Providence, Rhode Island. Dr. Levy is a founding member (2002) and a member of the Executive Committee of the Surviving Sepsis Campaign, a global initiative to improve the care of patients with severe sepsis. He is the lead investigator for Phase III of the campaign, the goal of which is to facilitate adoption of evidence-based guidelines for sepsis management into clinical practice and reduce mortality in severe sepsis by 25% by 2009. Dr. Levy is Past-President of the Society of Critical Care Medicine (2009). Dr. Levy’s current research interests include biomarkers in sepsis, end-of-life care in the ICU, and knowledge translation. He has authored over 130 peer-reviewed articles and book chapters. He is the co-director of the Ocean State Clinical Coordinating Center, which manages large, international, multi-center clinical trials in sepsis. Dr. Levy is very active in the field of quality and safety. He continues to serve as the representative to the National Quality Forum for SCCM and also serves on the advisory committees on Quality for the Blue Distinction program of Blue Cross Blue Shield of America. Dr. Levy has worked on several state-wide initiatives on quality, including Rhode Island and New Jersey, and has led a similar initiative for the New York City Health and Hospital Corporation in their quality initiative in catheter-related bloodstream infection and sepsis. He was recently appointed a content expert and voting member of the Hospital Workgroup of the Measure Applications Partnership (MAP) of the National Quality Forum and serves as a technical expert for the project Closing the Quality Gap: Prevention of Healthcare-associated Infections, which is part of the Evidence-Based Practice Center (EPC) program of the Agency for Healthcare Research and Quality (AHRQ).

Research Overview

Sepsis Therapeutics and knowledge translation Mitchell Levy MD is conducting work on performance improvement techniques, and has been working with The Surviving Sepsis Campaign to improve the survival of sepsis through a process of developing management guidelines, establishing a minimum global standard of care, and then producing an implementation model that facilitates the translation of the guidelines into a change in bedside practice. The Surviving Sepsis Campaign guidelines were developed and published first in 2004 and then revised and published in 2008 and 2012. He has conducted several trials, testing the impact of “sepsis bundles” and a multiple-faceted intervention in facilitating knowledge transfer. In partnership with the Institute for Healthcare Improvement (IHI), two sepsis bundles were developed in 2004, and a multi-faceted performance improvement project (Phase 3) was launched in North American, Europe, Asia and Latin America, the results of which were co-published in Critical Care Medicine and Intensive Care Medicine in January, 2010. The final analysis of 30,000 patients in the sepsis database, demonstrating a clear association with higher compliance with the measures and lower hospitals mortality was also co-published in Critical Care Medicine and Intensive Care Medicine in January, 2015. Through funding from a Moore Foundation grant, an initiative, “Early identification and management of Sepsis on the medical floors” began in early 2014 and completed in August 2015. Knowledge transfer projects now include the introduction of a procalcitonin-guided antibiotic de-escalation as a component of antibiotic stewardship, in Rhode Island Hospital. A Sepsis biobank study is also underway with Dr. Levy and Dr. Elias (Dean of the Medical School) evaluating the timing and identification of several key cytokines during the immunosuppressive phase of sepsis. Predictive Models and Severity Scoring In Severe Sepsis Mitchell Levy MD (Division Chief) is examining large data sets of severely septic patients (New York State sepsis database) to determine the viability of a disease-specific risk-adjusted mortality model in patients with severe sepsis and septic shock. Using recursive partitioning and regression analysis, statistical correlations with these features are being identified. The aim is to generate a predictive model incorporating these factors to identify mortality risk. These efforts are in collaboration with Stan Lemeshow and Gary Phillips, from Ohio State University School of Biostatistics.

Research Statement

Current Research projects: End of life care in the intensive care unit (ICU): developing interventions for measuring and improving the quality of care at the end of life in the ICU. Sepsis: Improving the quality of care that patients with sepsis (severe infections) receive in the ICU Surviving Sepsis Campaign (executive committee): Changing clinical practice: developing models that help bedside clinicians utilize the most recent research results to improve patient care Computers in the ICU: Testing new computerized clinical information systems for the ICU. Outcomes reporting: Using databases associated with computerized ICU systems to measure and report outcomes of critically ill patients. This includes tracking the impact of protocols on care and the use of clinical decision support for improving care. NY State Department of Health to analyze and publish NY State public reporting with access to a database of 45,000 patients. Ongoing chairing of conferences on the Early Identification of Sepsis on the Wards with the Centers of Disease Control since 2015.
